Request for External CE Credit

Use the form below to request continuing education (CE) credit for training provided by agencies other than the State Library of Iowa.

The form should be completed within 45 days of participating in/viewing the training.

• You have the option of uploading a certificate, or answering some questions about your training.
• For conferences, please include a copy of the conference program as an attachment – indicating which session(s) you attended – so we can award the appropriate number of hours.
• Note that you may claim 5 hours of CE credit for required city government training during your three-year recertification period. (This limit applies to programs such as CPR, defibrillator, and blood borne pathogen training.)
• There will be a maximum of 15 credit hours for any one learning activity (conference, course, etc).

The CE Team will review your submission and add the hours directly to your IA Learns account (or we'll let you know if we have any questions).
